There are some important requirements you should be aware of before school starts.

Pre-K, Kindergarten, 6th, 9th Grade

These students MUST have an Illinois school physical and up to date immunizations. Until completed, your child will not be able to attend school. Copies of health examination forms are available from your healthcare provider.

Kindergarten, 6th, 12th Grade

Proof of the following is required BEFORE your child may start school:

• Illinois Physical Exam
  • Diabetes screening
  • Lead screening
• Vision exam by Optometrist- Kindergarten and students new to IL schools
• Up-to-date Immunizations
  • DTP, Polio, MMR, Varicella, Hepatitis B- 6th grade, Meningococcal- 6th grade & 12th grade
• Proof of Residency
• Birth Certificate
   

Sports:

All students participating in sports must have an athletic physical.
